Membership Agreement

 As a member of the Fit Box you agree to abide by the rules, regulations, and privileges of membership as set forth, as provided by Fit Box LLC, or posted, from time to time to be set forth or posted by Fit Box LLC.  It is further agreed that all rules, regulations and privileges, facilities, services hours, rates, dues, member’s fees and charges are subject to change without notice at the sole and absolute discretion of Fit Box LLC.  You as a member will not sell, assign or transfer this agreement or membership or any other right of privilege obtained from Fit Box LLC.  Failure to follow these policies will result in termination of your Fit Box Membership.

Only water is permitted in the fitness center.  NO other food or drinks are allowed. No chalk is allowed in the fitness center. Members are required to wipe down the equipment after use with the towels and spray bottles provided. To conserve waste, please use one towel during your workout to wipe down equipment. Used towels are to be placed in the metal baskets on the floor. Do NOT drop or bang machine weights and free weights, including barbells, kettlebells, dumbbells, or plates on the floor. Members will return all plates and free weights to the racks after use. All members must be 18 years or older, or if under aged, they must have the written permission of a parent or guardian to use the fitness center.  Anyone under the age of 16 must be accompanied by a responsible adult member. Fit Box LLC is not responsible for lost or stolen items in or around the facility. Members are expected to refrain from entering areas marked as “Employee Only” or “Private” except in cases of emergency. Members may only use personal audio devices with headphones; public broadcasting is prohibited. Television volume will be kept at a moderate level and televisions will be turned off after use.  IF you are the last one to leave, please conserve energy and ensure all televisions are turned off.  The lights will automatically turn off. Members are expected to be respectful of Fit Box LLC members and neighbors and refrain from obscene language, loud noise or excessive cell phone usage. NO smoking, e-cigs, tobacco products, or drugs on premises. Use of the red wall phone is for emergencies only and will only dial 911.  Do not block or obscure the red emergency phone or the fire extinguisher. Members may not bring their own fitness equipment, except for yoga mats, to the Fit Box. No solicitation on premises. 

Proper, freshly laundered gym attire is required; shirts, gym shorts, sweatpants, yoga pants, leotards, and/or tights.  No street clothes allowed.
Shirts must be worn at all times within the fitness center.
Closed toed athletic shoes must be worn at all times. No sandals, flip flops, boots, bare or stocking feet.  Shoes are expected to be free of excessive dirt and moisture.  During inclement weather, please change into athletic shoes after entrance into Fit Box.  

Fit Box LLC reserves the right to close any or all of the fitness center for construction, repairs, and/or maintenance without compensation.  Fit Box  LLC also reserves the right to close any areas the staff/owner considers dangerous to use.  Closed reservation of the facility may occur for classes and events without compensation.  Fit Box LLC will take reasonable steps to inform members of such closers.

Members must use the access “key” assigned to them upon membership to access the fitness center each time.  Do not lend out or share your “key” with others.  Doing so will result in immediate termination of membership without reimbursement and incur a $50 fee.  Lost or stolen “keys” must be reported as soon as possible to help@ccfitbox.com.  A replacement “key” may be purchased for $10.  If your “key” breaks, return it to PO BOX 156 Central City, IA 52214 or notify help@ccfitbox.com and a replacement will be sent at no cost upon receipt of the broken one.

Members may NOT bring guests. Doing so will result in immediate termination of membership without reimbursement and incur a $50 fee.  Non-Members participating in a class or special event will be required to pay the respective fees to Fit Box LLC prior to attendance.

​Request to cancel your membership must be provided in writing with the access "key" included. Requests can be placed in the drop box located inside Fit Box or mailed to Fit Box LLC, PO Box 156, Central City, IA 52214. Fit Box is not responsible for "keys" lost in the mail. You may be charged a $10 fee for a lost "key."  Cancellation may take up to 30 days to process and reimbursement of any outstanding membership fees will be mailed by check to your address on file as long as account is in good standing and “key” is returned.   Unreturned “keys” will be subject to a $10 fee upon cancellation.

If the automatic payment of the membership fee is refused for any reason, your account will be assessed a $30 Non-Sufficient Funds (NFS) fee for each occurrence.  Access to the Fit Box will be suspended until your account is paid in full.  Accounts incurring more than two NFS fees in a twelve month period will automatically be terminated and assessed a $50 termination fee. Please note, if you close or change your bank account please contact help@ccfitbox.com as soon as possible to avoid a NFS fee.

The parties to this contract agree that all disputes arising out of or in connection with, or in the application of, any of its provision shall be finally resolved through arbitration under the administration of Federal Arbitration, Inc. and in accordance with its Rules for Arbitration.
Your payment of membership signifies your acknowledgement of receipt of this Membership Agreement and your acceptance to adhere and abide by these terms.
